Assist two Durango School-Based school-based health center with continued expansion and financial sustainability.

Continued support for Kids First Health Care school-based health centers located in Adams 14 School District.

Provide elementary school students with access to primary health care, preventive dental care and mental health services through two school-based health clinics. APS and Aurora Mental Health Center (AuMHC) have partnered for over 25 years to provide mental health services to students during their school day, within their own school buildings. This request will support two full-time mental itinerant mental health therapists.

Summit Community Care Clinic is requesting continued financial support for the integrated primary care, reproductive health, behavioral health and oral health services provided at the four school-based health centers in Summit County Colorado.

Create and develop a new playground environment that is conducive to increased physical activity at Metz Elementary school that also will be used heavily by the surrounding community.

Continue and expand activities of 22 Family Resource Centers to increase healthy living and health coverage activities for thousands of vulnerable Coloradans annually.

Implement a community active play area located at the K-12 school based on the Wiley School District and Colorado Center for Community Development Learning Landscapes plan.

Build a comprehensive fitness trail and community park, which will be available to the surrounding underserved communities seven days a week, to address the lack of recreational amenities and opportunities in Teller County. The project will provide multi-generational access to recreation and fitness, serving all ages and abilities.

Reduce childhood obesity by improving student's knowledge of nutrition, willingness to try healthy fruits and vegetables, and increase the amount of fresh fruits and vegetables students consume. This will be accomplished by deepening and sustaining existing activities and through a strategic expansion of efforts to increase the amount of local foods made available in Montezuma County.
